Medical Record Fees

NOW THEREFORE BE IT RESOLVED, the Board of Directors of Delaware Township Volunteer Ambulance Corps agrees to adopt the following fee schedule for record reproduction and that these fees will be effective immediately upon signing of this resolution:

Flat Fee for supplying records requested by a District Attorney – $ 25.20

 Flat Fee for production of records to support any claim under Social Security of any Federal or State financial needs-based program - $31.94

Flat Fee for supplying records requested by a Patient Representative - $25.20
    **All fees include standard postage, additional postage will be charged directly to the requestor**

***THERE ARE NO FEES FOR ACCESSING YOUR OWN MEDICAL RECORD IN PERSON***
